Going home

Going home from hospital after a stroke is an important step for all stroke survivors. Each person will have different needs at home and go home to different types of support. If you have been in hospital, a number of things will happen before you go home. These will depend on what has happened to you as a result of the stroke and might include:

  • The doctor looking after you letting your General Practitioner know you are leaving the hospital and providing a summary of your care in hospital and ongoing needs.
  • The occupational therapist might go to your home to ensure it is safe for you.  You may be provided with equipment to improve your independence and safety.
  • You will be advised by your doctor regarding driving and/or working following a stroke.
  • The social worker will give you a list of people in the community who may be able to assist you.
  • Follow up appointments will be made for you with the people you will need to continue seeing after your discharge.

If you have any concerns about going home, it is important to talk about these with the team. Keep asking questions until you are comfortable with the answers.  View the Long Term Recovery booklet Image (.pdf 1MB)
