Palliative Care After Stroke To access the Implementation Guide please click here. ‘Improving access to palliative care for people who have had a stroke and their carers' Louise Weir, a Stroke Nurse Practitioner at the Royal Melbourne Hospital spoke at the Stroke Society Australasia Scientific meeting in 2010. She presented on the following topic: ‘Improving access to palliative care for people who have had a stroke and their carers’. Louise spoke to Carmen Lahiff-Jenkins, Managing Editor of International Journal of Stroke. The available podcast talks about the background and aims for the palliative stroke project piloted at Melbourne Health. There is also a discussion about some of the key predictors of mortality for haemorrhagic and ischaemic stroke patients. It provides a review of the principles of palliative care and a discussion of why this is such an important area in stroke management. It discusses the development of the Stroke Palliative Approach Pathway (SPAP) and other resources that were developed to support the use of the SPAP. The podcast can be used to support the wider implementation of the Stroke Palliative Approach Pathway in other health services.
