The development of the Stroke Care Pathway: a resource for health professionals was supported by an Expert Advisory Group.
The Hospital Peer Support Program (HPSP) was developed and piloted by the National Stroke Foundation (NSF) as a strategy to provide timely access to psychosocial information and peer support for stroke survivors and carers as they transition from hospital to home.
A comprehensive quality improvement program offering outreach visits by NSF staff using interactive educational formats linked to audit and feedback and local consensus processes.
